The applicant, in submitting this application form: 1. Acknowledges that he/she has took notice of and agrees with this "Statement of Agreement" and
the Privacy Policy 2. Certifies that he/she fills this form on behalf of the ISM Manager, agrees with the requirements of this form and certifies that he/she has completed it truthfully. 3. Permits the Bureau Green Award to obtain information about the ship's performance from a third party. 4. Will, on request, share documents for the relevant vessel(s) in the event that additional information is required for the purpose of assessing the ship(s). 5. Ensures that the ISM Manager will cooperate with the verification process including the onboard verifications for assessments of criteria 3 until 10 of the Green Award Ship Waste module.
6. Understands that the validity of the verification results remains the sole discretion of the Green Award Foundation based on their assessment on the information provided at the time of application. The results or its validity may be revoked if other information is known beyond the time of the application process. 7. Acknowledges that the ISM Manager will receive a Green Award Ship Waste module Verification Report signed by the Green Award Foundation and, on their request, all relevant information regarding the ship received or gathered from parties other than the applicant.
The Bureau Green Award 1. Is obliged to keep all information received confidential in line with the confidentiality clause. 2. Will, on request, make all relevant information regarding the ship received or gathered from parties other than the applicant,
available to the ISM Manager. 3. Will, upon verification, provide the ISM Manager with a Green Award Ship Waste module Verification Report signed by the Green Award Foundation.
Confidentiality clause 1. The Bureau Green Award will be obliged to keep confidential all information concerning third parties which it receives as a result of processing applications and granting certificates except when required by the ISM Manager or where required by law. This does not apply to ship name and IMO numbers of the applied ship(s), their results of verification based on the Green Award Ship Waste module, and the date of its validity, which are provided to third parties (such as port authorities). 2. The Bureau Green Award will be obliged to take all necessary measures to ensure that the Foundation and its staff members maintain confidentiality about all information which has come to their knowledge as a result of the application process.
